A practical tool for community aged care providers

This tool was developed to guide and support providers in the aged care sector to deliver accessible and inclusive care services for older people who are at risk of homelessness or are homeless. It contains six sections of resources, activities, case studies and practical strategies to support staff in their professional development to work well with older people. The resource provides links between the aged care sector and the housing sector and compliments the commonwealth governments aged care quality standards and the 2017 diversity framework.
